I hereby understand and agree that Hockey Sense, LLC, its staff or facilities used shall in no way be held responsible or liable for any loss, damages, claims or actions (including costs and attorney's fees) for bodily injury and/or property damage suffered by the above named participant while attending Hockey Sense sessions, to the extent permissible by law. This agreement shall include indemnity against all costs (including without limitation, reasonable attorney's fees and court costs), expenses and liabilities incurred in or in connection with any such claim or proceeding brought thereon and in defense thereof. I have read and understand this form. I voluntarily sign it and hereby give permission to Hockey Sense LLC for emergency transportation and/or treatment in the event of illness or injury. I hereby accept responsibility for the payment of any emergency transportation and/or treatment and other costs related to participation the Hockey Sense sessions. I further certify that my child has medical insurance, is in good physical condition, and has no medical or physical conditions that would restrict his/her participation the Hockey Sense sessions. Additionally I grant permission to Hockey Sense, LLC to use images (photo or video) of my child on the Hockey Sense website or social media accounts including (but not limited to) Twitter, Instagram and Facebook. Hockey Sense, LLC, its staff or facilities shall not be responsible for claims or demands related to these images including invasion of privacy, infringement of right of publicity, defamation and any other personal and / or property rights.
